Brushy Mountain sells them. http://www.beeequipment.com
They don't include a picture in the on-line catalog.  Maybe because it is 
pretty easy to duplicate.
I made my own because I needed one for medium frames.  I added a bunch of 
things to mine, a hidden bottom panel that covers a double screened bottom 
board, double screened ventilation hole and a queen excluder under the 
observation area.  I can't post a picture here.  I'll e-mail you directly.
