Hi, all, I have a mom with a healthy gaining 6 week old, contacted me because her nipples have been sore since breastfeeding was initiated. She states the pain is worse between feeds, and feeding actually provides some relief. She describes burning and tenderness which sounded very much like thrush. There is no pain on latch and good weight gain, mom doesn't think it can be a latch issue. (Hmm. I will have her check the baby for a short frenulum tomorrow though!) No cracks or fissures and no cold sensitivity or blanching. She has not been using any creams or self-treating, she only called me this week because a friend told her she shouldn't still be hurting. After speaking with me, she decided to obtain gentian violet and did an application each day for two days. She spoke with me again today and said that she'd decided to go to her OB because she felt worse, the GV didn't seem to help, in fact, today she felt sore while nursing which was a change. Doc made some wisecrack about her purple nipples, and diagnosed "nipple inflammation" and gave her a script for Domeboro Soaks. I still think it is most likely thrush and I'm hoping the two applications of GV will provide some relief, which now she will probably attribute to the Domeboro. Baby had no sign of thrush, but had one application of oral GV (this was what mother decided). Anyone ever heard of using Domeboro Soaks for sore nipples? I think it is an astringent and could cause drying and cracking. Just what this mom needs. Any other thoughts here? Mom is very motivated but discouraged. (Hmm, now I'm really wondering about that frenulum.)
