To : Jan Re : DHE-45 Sorry, but its really unlikely that DHE-45 is going to work in this patient, and it was an especially poor choice to use in this mother. A little morphine or sumatriptan (Imitrex) would have probably been better. It should be obvious that the ergotamine is suppressing lactation via its powerful inhibition of prolactin production. Unfortunately I do not know, and cannot yet find, the length of suppression following ergot exposure, but I would assume that prolactin production could be restimulated with enough stimulation and some Reglan (30-40 mg/day). But I would imagine the earlier the better. With other ergots (cabergoline), after 24-48 hrs of therapy, prolactin is continually inhibited thereafter. Regards Tom Hale
