A Grav 3 Para 1 woman delivered a 7 lb 5 oz baby via spontaneous vaginal delivery. When seen in the hospital she had pain while nursing and had sore nipples related to poor latch-on. Was advised to take baby off breast if had pain with nursing and relatch baby. She was seen once in the hospital. I did not see her because I was on vacation. On followup phone calls, she was unable to be reached and never called to state she was having any problems. Approximately 10 weeks later, I was called by this mother. I returned the called and was told she had stopped breastfeeding because baby had only gained 6 oz over birth weight in 10 weeks. Her primary physician had the mother give abm via nipple. Breastfeeding was eventually discontinued. If I had been notified of situation before mother discontinued breastfeeding, perhaps I could have made a difference. I could have inquired on frequency of breastfeeding, length of nursings, hydration status of baby, had her come in for office visit to determine if baby had good latch-on and good suck. I'm sure I have missed some other things I could have done, but what are your suggestions. What are some of the other reasons this occurred? Could it have been a decreased fat content in the breast milk? I wish I was informed, but credibility as a profession still is not there. Would we go to a medical doctor with an animal illness or to a veterinarian with a human illness. I bet not. So if we have more education in this field, why are we not consulted? I know this is a problem all over, so I am just venting my frustration. I also look forward to the digest daily. The information is benefical. The networking is great.
