    I tell the mothers with whom I work that they should let the baby stay on
the first breast as long as s/he is actively feeding, then offer the second
breast.  If the baby wants it, good enough.  If not, mom is to give that
breast with the next indication of hunger. (I do tell the moms to expect that
the baby may eat more often if s/he follows this pattern.)  Particularly if
the baby has had a difficult time attaching or has been sleeping for hours, I
don't like to interrupt when s/he finally does begin to nurse--"don't mess
with success," as they say.
