Our policy is at 5% wt loss in 24hrs, the patient gets a visit from the IBCLC to evaluate the dyad. We start pumping/hand expressing mom and give to baby if at that point if there looks like any kind of latch issue. At 10% wt loss in 48hrs we supplement either w/ ebm or formula per amount recommended by the neonatologist (usually 15-30ml) after feeding at the breast.
