I think reviewing where the problem 'about' happened at. Is it the parents poor handwriting or the nurses grabbing the wrong bottle. Once you can pin point the problem it may make an easier fix. 

At our hospital each mother is provided with a sheet of labels that the parents right on but each child is provided with a certain color. So each container of milk that comes in is labeled by the parent but once handed over to the nurse accepting the milk the nurse places a colored dot on the top of the container and the milk is placed in a bucket that has the same colored tape on it and then prior to giving the milk 2 nurses verify it.
