You need to see what the baby is doing at this breast. It's not unusual 
for a baby to suck or latch differently on either side based on 
differing nipple anatomy, or baby's issue. Torticollis is one reason 
baby might have difficulty on just one side, a mild to moderate tongue 
tie is another. If one nipple is slightly flatter or less easy to grasp, 
a tongue-tied infant may have difficulty with that one breast while 
doing fine at the other.

Perhaps she can tell you what her baby's sucking feels like just before 
the pain starts. Her description can give you a clue as to what's 
happening inside the mouth. I would guess that this pain is caused by 
excessive compression of her nipple on this side that's disrupting blood 
flow (hence the warm washcloth helped), but it's only a guess without 
seeing what's happening.

Is there an IBCLC or an experienced LLL Leader in the community that can 
see her, or are you the only breastfeeding helper in town?
