Nikki, I attended a talk by Dr. Jayne several years ago and she discussed large/long nipples. Her suggestion was nipple shields until the baby grew into mom's nipples. She stated that the nipple can be compacted into the shield and be comfortable for the mother. The way a nipple comes into a nipple shield is not the same as in a pump shield. Women have done some pretty amazing things to their nipples and tolerated them fairly well. (piercing, bras, pasties :-) ) I think it is worth the try. If a mother is finding the shield uncomfortable than this is not the option for her. The same concept applies to the little preemie baby and mom putting on a small nipple shield. I have found the mothers tolerate this fine. Hope this helps.
