In Australia, I believe it would breach the Privacy Act to require this consent (though not, of course, to seek it). In addition, if women wish to be seen completely anonymously in any healthcare setting, they may choose that. The health practitioner must then warn them of the risks of such an approach, and make them aware that they may not be able to seek insurance reimbursement.
