Liz writes about a baby who shows signs of difficulty breathing while at the
breast, but who can drink pumped breastmilk from a bottle without trouble.
Liz, have you observed a bottle feed with this baby?  A baby who needs to
breathe through his mouth may be able to handle feeding by bottle if the
nipple just lolls around in his mouth, not requiring him to seal off his
oral cavity completely, so he can still get air while drinking.  The breast
requires a pretty complete seal by the lips and jaw, while a bottle doesn't.

It does sound like something in the nasal passages, but what?  
At any rate, it's a good idea for this mother to be expressing milk at
present, because the baby's technique sounds like it would put her at risk
for a sudden drop in production due to lack of effective mechanical
stimulation / good drainage by baby if she left it all up to him.
