Margaret - You asked about a posterior tongue-tie - how to spot. I had not been looking for this until read the article you cite. Have found just one and yes it was difficult to assess and to clip. First physician stated there was no problem - it is very tricky to see. Second physician listened when I said "you have to elevate the tongue with your finger at its base in order to visualize". He did this and noticed the thin membrane and clipped. The mom had immediate relief from her ongoing sore nipples. What made me pursue the posterior tie was a slightly bunched appearance to the tongue and occasional hints of central tether as baby tried to extend and elevate tongue. Mom was experienced bfdr with good technique. Hope this helps.
