We include date of the call, mother's name, delivery date, doctor, reason for call. It is very brief. We also have a full page form to record all the details -- why the mother called, baby's stats, what the mother says and what we recommend, whether we referred and to whom. That goes into the mother's folder, which is kept in the LC office.
