I've searched the archives and found that this question has arisen several
times, but can not locate if the answer was posted to the list itself.

2 weeks ago a "feed and grower" premie was fed milk from another mother in
our nursery. This current error has accelerated a policy making process
that was already being debated about how to prevent/mimimize this possible
error, and what to do if a feeding error did occur.
Departments of Risk Management and Infection Control are now getting
involved and want a policy in place by Monday.

Any of you who have specific hospital policies addressing this issue?
I'd appreciate it if you could possibly share a copy with us!
