Complicated.  The burning pain and rash on the trunk, extending anterior
and posterior, reminds me from the description of shingles (varicella
resurgence).  The redish/purplish discoloration could be consistent,
though the lumps are a little puzzling, but could be consistent.  Though I
would not expect to see problems on the other side.  Is the rash along a
dermatomal distribution?

I suppose it could be a mastitis that has spread along fascial planes,
which could be troubling, especially without response to the antibiotics.

Not confident in either diagnosis though.  Wish I was there to see the
patient.
