Hi Beth Try looking at page 9 of this pdf. File http://www.rcp.gov.bc.ca/Guidelines/General/Master.NutritionPartII.PremBreas tfeeding.October.2001.PDF for a picture of dancer's hand As for the shield have you tried inverting the shield halfway before placing on the nipple? Once you have it placed over the nipple with the nipple right in the centre of the shield, you can turn it back out to its original shape. This allows for more of the nipple areola complex to fill the shield. I find this trick invaluable! Also have you experimented with different sizes? See Medela's site for a picture of what I mean although I find I turn it out even more than in this picture http://www.medela.com/NewFiles/nipshield.html "Sit the shield on the nipple with the brim turned up like a hat. Then smooth down the edges. This will help the shield stick better. Moistening the edges will also hold it in place. Some mothers have larger nipples than the small shield can accommodate. Try the next largest size. However, observe carefully to see if this size works for the baby. A shield will not solve all problems, and sometimes other equipment may be appropriate."
