Janet, It does sound like the NICU you are describing is taking a very conservative approach. Perhaps they feel that the nurse must stand by while a mom is giving K-care to a baby they feel is unstable. If baby stays in the crib or bassinet with all the monitors functioning, the nurse can attend to her other charges! It sounds like one of those walls that you have to keep chipping away at a little at a time. 50 as a bottom limit for blood sugars? And I complained when they went from 40-45 where I was. They also cited some "new research" which I have not seen. One tip-be sure the nurses are warming the baby's heel for the stick the same as they would do when drawing larger amounts of blood as for the infant screen. It is possible to get the drop for a glucose test without warming, but I ahve seen significant differences in the results with readings 5 or more points higher when the heel was warmed first! It can make the difference between supplementing and not supplementing. Policy where I was stated that for a slightly lowered range (can't remember if it was 35-45 or 40-45) without overt symptoms, baby was to be nursed, then blood sugar rechecked a half hour later and if still low, then supplement given.
