I think you're on the right track. The study doesn't mention causes for decreased effectiveness of LAM for child spacing, but it sticks out a mile. With increased urbanization, ABM becomes more readily available and mothers are more exposed to advertising for ABM. Even "occasional" bottles means a quicker return of menses for many women.
