In many states, early intervention evaluation is free and includes an OT
evaluation.  In NY, LCs can refer for an EI eval, as can the physician
or the mother herself.  Treatment is free in NY, and in many other
states (some states have a cost sharing plan, I know Texas does, parents
pay a percentage based on their income).

The only downside of going through EI is that it can take a few weeks
for the evaluation to be scheduled and services to start.
