To comment on Denise's cultural trivia:
 (Aside:  am I right in my thinking that here in Aus a physician is the term
used to describe a doctor who specialises in medical problems, as
opposed to a surgeon or an obstetrician).

Here in South Africa, a doctor is a "doctor".  Most people see a GP (General
Practitioner) and refer to him/her as "my doctor" or "my GP".  A "physician"
is a doctor whose specialisation is medical issues.  A few years ago few
only sick babies/children were seen by paediatricians.  Now all
babies/children seem to see "their" paediatrician from birth.
