They don't directly cause flat nipples, but what I have seen is areolar edema secondary to lots of IV fluids and they really load mom up on the IV fluids with epidurals. Perhaps there is a more direct connection with the epidural drugs rather than the amount of fluid, but I think it is still areolar edema rather than actually flat nipples.
