jennifer i hesitate to say this becoz the mom has her hands full with a 20 mo old and 5 yr old but....she needs to protect her milk supply. baby seems to be gaining now but they are at risk. did her milk 'come in' within the usual time frame? do her breasts seem full, and then soft after bf? usually when newborns fall asleep quickly at breast I find the milk supply is low or the latch is poor resulting in a poor milkflow. watch the progress closely and maybe even do a milk transfer on a scale. i'm sure you checked but is the baby tongue-tied or any such issues.? some red flags for me would be the 11 oz wt loss from birth to discharge, probably less than a full 48 hr time frame, and the "urine was dark yellow or orange the first 4 days." and also the current feeding behavior, which should be pretty settled by day 6. this is all i can think of this late.
