Jennifer, Low weight gain coupled with infrequent feeds seems worrisome to me; a good friend of mine sort of had to "force" the issue with a couple of her 6 children when they just didn't ask to eat often enough and weren't gaining well. I am assuming that available milk supply has never been an issue for your mom, but that production has been set by baby's feeding pattern. Can we rule out covert scheduling and/or sleep training? If there is any way that you can observe a feeding (and do pre- and post-feed weights), that would be ideal and fill in some informational gaps that could help narrow down the issues. There is the remote possibility that something about feeding is simply not comfortable for baby, causing her to eat mostly to survive as opposed to thriving. Oral anatomical variations are one possible discourager. What do you know about baby's wetting and stooling patterns? It would also be interesting to know if mom's milk looks "thin" or creamier. There is the possibility of trying to influence the fat content with dietary manipulation such as flaxseed oil supplementation. If volume is a question, perhaps a galactogogue might help things along as an intermediate plug.
