How is the mother positioning this baby?  Ulcers on the medial aspects
of the nipple shaft can be from friction from the baby's tongue if the
latch is too shallow or tongue elevation is restricted by a tight
lingual frenulum if the mom is using a cradle or transitional hold.

Try an asymmetric latch, with the lower lip and tongue tip as far away
from the base of the nipple as you can get it and still allow the baby's
upper lip to get past the nipple.  (Am I being a broken record here?)
