I'm sure the phone nurses have guidelines when other types of calls need to be passed on. They wouldn't try to "talk through" a person with concerns about chest pains, for example. They can be helpful with some basic concerns, but need to have guidelines as to when the mom needs to be referred to a specialist (the LC). It concerns me that the attitude is that "any" nurse has the info to solve all breastfeeding concerns. Certainly she (or he) would be able to give guidelines on milk storage. A mom asking if Lansinoh or Pure-Lan needs to be washed off before nursing can get that info from a phone nurse, but is a triage nurse likely (or have the time) to go into why the mom is using it. If she has cracked, bleeding nipples but believes it's normal and she just has to "toughen up", she needs more than just the simple answer to her initial question. Certainly BAB is a good source. Our "Nurse-on-Line" nurses have it. They do the routine 2 day post d/c follow-up calls for all our discharged OB patients, but are taught to refer moms with breastfeeding problems to us. (It helps that many of these nurses have worked OB before.)
