In response to Kathleen's question regarding charting: We have lactation charting forms in the mother's chart that the IBCLCs use. It is behind the tab marked 'lactation' for ease of use. The 2 part form is used for the mother's entire stay as there is a place for the date, time and initials of the IBCLC for each visit. When the mother is discharged, the form is torn down with one part going to medical records with the rest of the chart and the other part going to the lactation office for follow up calls and for continutity of care when there is a follow up appointment. We have just done another revision of it and I am eagerly awaiting the new form!
