Interesting case.  I'm surprised that
this mom was able to conceive and carry without intervention, do you know if she
had "treatments" that enabled her to do so?  This condition, over
simplified, means that there is a blockage in the adrenals that prevents one
hormone(A) from breaking down into the next hormone(B). This causes too little
of "B" and too much of "A". Hormone A is testosterone.  I can certainly see
that this might have an impact on lactation, but don't know the relationship
between prolactin and testosterone.  Perhaps inhibitory? Certainly an
estrogen/testosterone imbalance.  If this condition was present, mildly,
from childhood, perhaps it prevented her from developing normal breast
tissue?  I suppose a call to an endocrinologist is best, can you talk with
hers?
