Although the books say that the effects of dostinex are irrevesible, I have had two cases where we managed to re-establish the milk supply. The half-life of this drug is 40 hours so you have to take that into account. In one case the mom had not taken the last pill, but in the second case the mom had taken the whole course and her breasts were flaccid and back to pre-pregnancy size. Two weeks pp, the bby developed a deadly virus and was rehospitalized in critical condition. The only thing that could help stabalize him was - you guessed it- the magic golden liquid!! He was TPN for long enough to get into a good pumping regimen and get adequate amounts of breast milk. The head doc gave her Domperidone to give the relactation a jump start, and within 4 days we were getting surprisingly adequate amounts of milk!! Whether or not she achieved full breastfeeding for an extended period, I doubt. She was a single mom who had to go back to work quite quickly which is why, when the breastfeeding didn't go well in the first place, she had given up. She did get him through his illness and when I last heard was still pumping enough to sustain his needs. She was planning to geet donor milk from a close family member. However, that was only about a week after his release from the PICU. My work load does not permit me to keep close follow-up unless the patient initiates. So it is worth a try!! Good luck!!
