Maybe an obvious question and already been asked, but has the mother or
father tried giving the mothers milk themselves?  Just to see if the
childcare provider has some problem with actually handling the human milk.

And, just from my own experience, our daycare was very cooperative in using
the SoftCup Feeder, (except, once, when a sub was there and proudly
announced that she had got him to 'take a bottle nipple' and he drank 6
ounces at 11:00! I arrived at @11:30 to nurse and he was not interested in
eating then!  I cuddled for a bit and then pumped. I wasn't mad just annoyed
and explained that he was not allowed to have a bottle nipple.)
I had a 12 week family leave, so, my son started daycare at his 13th week.

I think the daycare has an obligation to try and get the situation to work,
without formula, perhaps they truly have but I would investigate further.
What if the baby had had some reaction to the formula?!
