Nipple or breast shields can predispose to mastitis because they interfere
with good emptying of the breast.

Breast pads don't directly affect how well the breast is milked.  If they
are harboring bugs they could theoretically be an inoculum, but the breast
would have to be vulnerable (broken skin, poor emptying) as well.
