The first rule is to feed the baby.  If the baby is not getting much 
using alternate feeding, then go back to bottle feeding, but change the 
rules a bit so baby has to open wide and actually suck for the milk.  
Check the archives for modified or paced bottle feeding.

Since mom has very little milk, there is little reward for babe to go to 
breast.  Perhaps mom can put baby to breast with a nipple shield with a 
feeding tube under it, when he's not starving hungry, perhaps for 
"dessert" after he's taken most of a bottle.  Lots of loving skin to 
skin helps too.

A lot of getting older babies to take the breast is in helping bring 
their own self-attachment reflexes to the fore.  Skin to skin, massage, 
and getting the baby's chin right in the breast often do wonders.

It's important not to unduly frustrate the baby.  Mom will need support 
to help him work a little bit, but not so hard that he refuses to ever 
try again.
