I think what would work would be continuous skin to skin (kangaroo) care. 
Mom and baby can cuddle for maybe 12 hrs, resting. Then mom can begin to 
express (hand or pump) and feed by spoon her colostrum every couple hours. 
When mom is expressing, Dad (or other close family member) can kangaroo. If 
the baby is not nursing in a day or so, a bottle can be used.
The baby #1 may very well have begun bf, but if the mother had not started 
offering and was not expressing but once per day, the chances were slim.
The main things are : early expression of colostrum, freqt expression esp 
once the milk comes in, skin to skin and offer the breast often -- but you 
knew that!
