I worked with two such moms for the last two days, in hospital.  Of course, neither of their babies were very acticely trying to nurse, to further cause them distress.  Neither moms had nipples that would stay shaped by hand manipulation, and the babies weren't latching and or shaping them either.
Finally we got moms to pump, getting ready for supplementation.  And the old dinosaur Egnell pump shaped this nipples so completely, that an hour later, both moms still had "good nipples."

Oddly enough, both babies got very vigorous just as their moms pumped and
went on to get their first really good latches (latches of 8 and 9) and successful breastfeeds.  The babies were 16 and 36 hours old at the time, and had totally different types of deliveries -- the younger one having had an epidural, and the older one a medication free delivery.  Go figure.  Were different pherenomes being exuded once moms nipples got shaped?  Was there essence of "successful breastfeed" in the air?  Did their Vitamin BF kick in?
