Have permission from the mom I am working with to post, as I am quite
stumped.

Baby is 4 wks old, has been very "gassy", starts to nurse, comes off the
breast crying and occasional green stool. Gain has been ok. Some signs
of Over-active milk ejection reflex.

Spoke to this mom about a week ago and discussed nursing leaning back
with let-down or nursing lying down. Also discussed nursing from one
breast per feeding. Had small improvement.

Mom called a couple days ago, symptoms worse. States she can hear milk
"hitting bottom" when baby nurses.

My guess was oversupply or hyperlactation. Went through the archives
and found lots of good info. When I read to this mom from the Lactation
Consultant Series about oversupply, she stated that that described her
baby to a "T".

We discussed nursing from one breast per 3-5 hour period, no pumping,
and non-gravity positions.

Got a call from her today. She says she is experiencing many forceful
letdowns with feedings. Baby is very unhappy and crying at the breast.
She tries taking the baby off the breast with letdown, however after a
couple of times, the baby will not go back on the breast. Has been
using same breast for a 4-5 hr period.

Should we expect the supply management to take a few days? I kind of
thought symptoms would improve atleast a little and not become worse.
All I could offer her were a few other anti-gravity positions.

Anybody have any ideas? What am I missing?

TIA,
