If we say that us opening white spots is not covered by the standards of
practice, then can we justify handing a mother a sterile needle and
educating her to preform the procedure herself? Surely by doing that we
are still "responsible" for her undergoing that procedure, even at her
own hand.

I posted a previous question about tubing infants in emergencies and the
response was that as an invasive procedure it was outside the scope of
practice for IBCLC's. I would have thought then that lancing a white
spot from a  nipple with a sterile needle would also be considered an
"invasive procedure".

What is the general consensus? Do we make a clinical judgement on the
availability and likelihood that the mother will be  referred quickly to
a qualified person for treatment?
