Marianne, this may be a language/translation discussion, or just the strange
language of medicalese.

When something is called "secondary" it means the condition is there because
of something else. So for example, secondary failed lactogenesis occurs in
mothers of late pre-term infants when the breast is not well-drained. (I
know I know there's other reasons too.)

Primary lactogenesis would be when the condition itself is not there
"secondary to" something else.
