I have only an anecdote to throw out about the woman who is running a fever
and has no supply.   About 6 months ago, I came down with strep and when I
got home from getting my throat cultured and picking up medication, my fever
was up to about 101 and my body was shivering with chills.   I tried to pump
(being a very good producer under usual circumstances) and I literally got
not one drop of milk, even though I was very full.   My sense at the time was
that the fever---and, particularly, the chills---were not allowing a MER to
happen.
