In addition to working on getting tongue to extend further, I would
recommend mom "wear" her baby skin to skin as close to 24/7 as her
situation will allow!  Minimum would be to have at least as much
"cuddle" time at breast as "work time".  I find even overwhelmed
moms can usually commit to the latter suggestion.  I have had many
moms who report that baby will squirm over latch on "out of the
blue" when they least expect it.  Even giving bottle or whatever
feeding method with baby's cheek against the breast if this is
workable can help.  It gets baby to associate the feel and smell of
mom's skin with feeding.

I wonder from the description of baby's behavior if maybe he is used
to the steady flow from a bottle ("x" amount of suckling always
gives "y" amount of milk until it's all gone).  It may be that when
flow from the breast begins to lessen, he gives up.  Has she tried
breast compression when he begins to show signs of getting "antsy"?
If giving a bottle, varying the flow can make it more like nursing
in at least this small way.  Is she wiling to try suplementing at
breast.  This would be the point in a feeding where one might "turn
on" a supplementing device.  I like to have it in place at the
start, but not flowing.  then it is less of an interruption to start
the flow when baby begins to show signs of getting frustrated.
Also, there was no mention of whether mom is pumping.  Not only will
this give baby EBM instead of formula, but it would lead to
increased flow to keep baby interested longer.

In trying to figure out what might help an individual baby, I like
to start by observing baby and asking myself. "What difference
between nursing and whatever way baby has been fed might cause him
to act this way?"  Then I look at whether there a way to reduce this
difference, either to make breastfeeding temoporarily more like the
way baby is currently fed or to meke the alternate feeding method
more like breastfeeding (usually a combination of both).
