Daktarin is probably the drug of choice for thrush in UK.  It is available for
topical use, and also in tablet form for systemic use.  I have been wondering if
it is the same as the drugs you US guys and gals have been talking about - I
still don't know!

We have all the same difficulties with doctors being unaware of thrush on
nipples, and sceptical about systemic use.  Those who have used it have had good
results.
