Michelle,
This case is what we would call a 'train wreck'(no disrespect intended), in
that so many factors that go against establishing lactation are present.
Your plan of care sounds right on. The mother needs to recover and rest and
eat well. The baby should be with mama always, if possible, like kangaroo
care. If he can latch and suckle I would use a supply line (sns or other) to
give the milk as much as possible. Bottle feeding when the supply line is
not used. This mother needs lots of support and realize it will take time to
bring in a supply, to whatever extent possible. Breastfeeding and nurturing
thru bf more important than full milk supply for this baby at this point.
