Dear Amy,

This is a very difficult position that you are in.  But, as a health care
provider, I believe that you would be obligated to report this type of neglect to
the state children's services.  It is obvious that this baby is at risk for
several things including lack of appropriate medical care and malnutrition.  I
do not believe that you can take your concerns to a physician who may or may
not be the baby's PHCP of record without written permission from the mother of
the baby.  If something happens to this baby, you may also be held accountable
because you didn't report this to the proper agency in your state.
