Lisa, am I correct in assuming that she has been taking domperidone along with the BCPs?  From what I have seen, taking the BCPs and domperidone together can make a difference, but it usually takes being on it for several months to be more beneficial than just getting on dom and starting to pump.  There may be some amount of benefit from her having spent the month on the combo, but I wouldn't expect it to be dramatic .  Everyone is different, of course, but if she is trying to get by without any supplementation, I would watch the babies very, very closely.  I wonder if the surrogate might be able to provide some expressed colostrum/milk for them.  I would expect her to need to supplement for quite some time, especially since there are twins.  Even with all of the options available now, that weren't around when I was nursing my adopted kids, most adoptive/intended moms in western culture need to supplement, at least to some extent. I would really try to keep your discussion with her as positive as possible.  Try to assure her that her milk is very beneficial to her babies' health, even if they need to have some formula, too, and also stress the benefits of nurturing at the breast. I can attest to the fact that, especially for someone whose road to motherhood has been long and bumpy, having something you can do for your child, that no one else can, is a very gratifying thing.  One adoptive mom once told me "You can give anyone's baby a bottle, but you only nurse your own!".
