The woman who coined the phrase D-MER for "depressive milk ejection reflex" has put together an on-line video describing it.

A key piece of this, remember, is that these women feel *absolutely fine*... except just before their milk starts to flow.  It's very much as if the oxytocin hits some part of their brain that causes really negative feelings, before it enters their circulation and gets to their breasts.  A brief interval after the negative feelings hit, the milk flows.  Every single time.

We're still puzzling it through, and anyone is more than welcome to help us puzzle!
