Hi Lisa

Yep!  I'd definitely consider trying a shield and seeing what the 
baby does with it - but only _after_ the milk starts to come in 
properly .... otherwise the baby will suck mostly on emptiness and 
wont get the reward he/she expects.  For now, I'd rely on frequent 
spoon-feeding of hand-expressed EBM and relaxed attempts at the 
breast, stopping if the baby becomes frustrated.  I also prefer the 
cross-cradle for this kind of latching difficulty, because the mother 
will have more control of where to place the nipple in the baby's 
mouth - thumb could be depressed above the breast to tip the (short) 
nipple upwards to stimulate the baby's palate as much as possible for 
the latch, and then released as it occurs so that the nipple/areola 
tissue can be drawn as far back as possible in the baby's mouth.  Good luck!

Subject: Short Nipples/small breast

Premission to post. I am working with a 24 hr old infant, vac. 
assisted vag delivery, roots really well tries hard to latch but mom 
has small breast and short, not flat, nipples. What do you suggest? 
Mom prefers the cross cradle hold but baby will suck few times and 
gets frustrated. I am wondering if the nipple/breast tissue isn't 
filling up the space in his mouth to trigger more sucking.Would a 
nipple shield help? She is able to manually express nicely. I am a 
lactation counselor learning as I go so your words of wisdom are so 
greatly appreciated!! Thanks in advance!
