I am working with a mom 10 days out from delivering a 33.2wk girl via primary C/s for pre-eclampsia.  Mom is quite good about pumping q 3 hours, including getting up at night, but is concerned over the Lt breast not producing as easily as the Rt.  She is getting between 35-60ml with each pumping (baby is getting 30ml NG every 3 hours).  Discussed that one breast is the better producer, etc, talked about natural ways to help increase supply (STS, heat/massage prior to/during pumping, rest).  Set goal amounts for each 24 hours which she was greatful to have as she was focused on the individual pumping amount vs collective amount.  She is keeping up with intake need at this time.  She has the 30.5mm flanges, is placing pump correctly, pump is functional, but she is only able to pump 1 side at a time because of her size/dexterity.  Talked to her about ways to do hands free pumping using sports bra with slits to slide flanges in.  Large breasts, flattish nipples, very soft areolar tissue.  Is not bleeding abnormally or crampy, has no hx of PCOS or thyroid issues.  Is this just a case of ineffective mammary tissue, or am I missing something?  She is using the hospital pump when here, and has a pump at home.  Any thoughts/suggestions would be greatly appreciated!!!
