Or perhaps a less-obvious posterior tongue tie or lip tie? Here’s Dr. Kotlow’s webpage, I’ve reviewed several of the ones he has under Breastfeeding Health: http://www.kiddsteeth.com/dental-topics.php <http://www.kiddsteeth.com/dental-topics.php>
One of his PPTs from 2012: http://www.laserdentistry.org/uploads/files/conference/2012/handouts/2012_handout_kotlow_aldbreastfeedinglecture.pdf <http://www.laserdentistry.org/uploads/files/conference/2012/handouts/2012_handout_kotlow_aldbreastfeedinglecture.pdf>
Here’s a YouTube video of him doing an assessment. https://www.youtube.com/watch?v=5opSbXvL7yQ <https://www.youtube.com/watch?v=5opSbXvL7yQ>
We don’t diagnose but we tell parents that we suspect a tight fenum or frenulum might be contributing to breastfeeding difficulties. We refer to a pediatric dentist in our area who trained with Dr. Kotlow.

about a month Mother is now only bottlefeeding EBM, which the baby
tolerates beautifully, he takes 3oz in 10 minutes or less. She is really
tired of pumping. When I suggested a (choke) nipple shield, she said she
had already tried it and that it "tore up" her nipples.
