There is a brand new product available from
Upjohn which releases estrogen locally only (company literature
insists no systemic absorption) from a Silastic ring . The ring
contains a three month supply of the estradiol. Of course, the
drug rep I saw in my office last week immediately added that the product
is not approved for use in lactation! However, if there is no systemic
absorption, it ought to be ideal for this indication. I am very eager to
offer this to my patients--just haven't had a lot of time to do so
since it's a new product here in the USA. (Lots of European experience with
post-menopausal vaginal absorption studies, per the manufacturer.)
Any one out there in LactLand who has had a chance to use this
in Europe or elsewhere? Up until now, BTW, I've prescribed small
doses of estradiol cream with good results. Systemic absorption from the
cream is negligeable after the vaginal epithelium thickns up, which takes
about 2 or 3 weeks. Lisa G. Matthews, M.D. FACOG, Stamford, CT.
