I just saw a mom with matitis.
Second one in 4 to 5 weeks.
Symptoms started with pain in right breast in middle of the night and a few
hours later a burning left nipple.
Right side did not burn but no other description. Left side had a few hours
of superficial burning then self resolved. No evidence of thrush in baby,
moms nipple looked OK.
Shortly after burning on left resolved mom developed body aches, 101+ temp
and redness on right breast.
The last episode of mastitis was on the right also treated with frequent
nursing and ten days of cephalexin.
I started this mom on cephalexin again for 10 days.
Baby nurses in a cross cradle hold in day and side lying at night. Baby
sleeps with mom "like velcro"
This is mom's second baby, no mastitis with the first.
I reviewed risk factors with mom, cracked nipples (no) fatigue (she has a 2
year old) stress (no) tight bra (maybe) baby does drain the breasts well but
better on the left, no symptomatic blocked ducts.
Any other ideas? Treat for 14 days?
