I worked prenatally with a surrogate mom who pumped prenatally just in case, and continued pumping following birth providing breastmilk for the infant for a few weeks until the adoptive couple returned to their country.   I thought of the baby and how wonderful it was for that child to receive his mother's milk for that time.  It is a different situation for sure and did elicit feelings for me about both emotional impact on both mother and child, but I shared these with a colleague and helped this mother with her pumping concerns just like any other mother.  I don't think this mom ever breastfed but we did discuss intramammary antibody formation, and the mother had no issue with it but wan't sure of the couples feelings at the time.  Hope this helps you to sort through your feelings should you need to counsel someone regarding this.
