The Diaper Diary  http://www.breastfeedingmaterials.com/products/diaperdiary is
in wide use in the United States and is my personal favorite for giving
mothers prenatally so they know what to expect the first week (and for
several weeks afterward, if baby is up to speed at the end of that week).  I
am wondering what is available for what output to expect from pre-term
babies (by weight?) who are progressing well and being fed only on human
milk.
